  • You will evaluate resident's medical history, needs, and support network.
  • You will chart and maintain documentation for residents.
  • You will design resident treatment plans.
  • You will develop and maintain a positive working rapport with all team members.Requirements:Nurse Case Manager (RN) Qualifications:
    • Registered Nurse in the State of Illinois.
    • No history of abuse or neglect
    • Current CPR certification.
    • At least three years experience in long term care.
